That's a Flashlight Eyed Creature from "F.K.O.". 

He lives in the Underground. 

He watched Ivan the last Leaf Headed Creature chased by 

a Multiple Armed Monster while he was running. 

And he also was in "Swan Meat" with the rest of these Creatures 

at the Feast.
